Cabanda Home Care delivers clinically required nursing and personal care services to eligible DVA Veteran Card holders.
Services Include help with medication, wound care, hygiene/ showering, dressing and palliative care. Assessing client needs and developing a care plan, providing care to meet assessed clinical needs, re-assessing needs over time, updating the care plan as necessary, and working with other health care providers as required.
This care is to help clients maintain independence and receive the care they need to remain at home. Who can access DVA Community Nursing Services
To receive community nursing services for an assessed clinical need, a client must have either a Veteran Gold Card or a Veteran White Card.
How to access DVA Community Nursing Services.
If you think you could benefit from Community Nursing services, you will require a written referral from an authorised referral source, which can be a:
- Your general practitioner (GP)
- Treating medical practitioners in hospital
- The person who will be managing your discharge from hospital
- Nurse practitioner specialising in a community nursing field.
