Personalised Care
Whatever your needs, we will support you with an individualised care plan created together with you and your family to help you continue to live in your home. This program is part of the Australian Government’s range of ‘care for older people,’ providing services that bridge the gap between the Commonwealth Home Support Program and Residential Aged Care. To be eligible for the Home Care program, prospective clients are required to be registered with My Aged Care.

DVA Nursing Care
Cabanda Home Care delivers clinically required nursing and personal care services to eligible DVA Veteran Card holders.
Services Include help with medication, wound care, hygiene/ showering, dressing and palliative care. Assessing client needs and developing a care plan, providing care to meet assessed clinical needs, re-assessing needs over time, updating the care plan as necessary, and working with other health care providers as required.
This care is to help clients maintain independence and receive the care they need to remain at home. Who can access DVA Community Nursing Services
To receive community nursing services for an assessed clinical need, a client must have either a Veteran Gold Card or a Veteran White Card.
How to access DVA Community Nursing Services.
If you think you could benefit from Community Nursing services, you will require a written referral from an authorised referral source, which can be a:
- Your general practitioner (GP)
- Treating medical practitioners in hospital
- The person who will be managing your discharge from hospital
- Nurse practitioner specialising in a community nursing field.

Cabanda Care provides (CDC) Consumer Directed Care which gives you flexibility and choice in the delivery of your care and services under your home care package. It allows more control in determining the types of care and services you receive, how they are delivered and by whom and when.

Switch Providers to Cabanda Care
Remember, you are not obligated to stay with your current provider. If the services you are receiving do not meet your needs, you can switch to another accredited provider, such as Cabanda Care. To transfer your Home Care Package to Cabanda Care, simply complete the letter provided below and send it to your current provider.
